PCN dashboard is now available on iPrevalence

Data for your Primary Care Network (PCN) is now available through our PCN dashboard here Primary Care Networks (PCNs) were established by NHS England to support delivering care around small groups of GP Practices in local areas. The dashboard can be used by primary care colleagues to ascertain prevalence rates for individual PCN Practices (as well as PCN groupings of practices) and compare these with other Practices and PCNs in their NHS (or ICB) areas. Historical prevalence data added for 12 long term conditions

Data for years 2019/20 to date have now been added to the iPrevalence dashboard. This means that trend data for the last 4 years can be observed for major long term conditions and diseases using NHS data from England, Wales, Scotland and Northern Ireland. Launch of the iPrevalence website & dashboard

The iPrevalence website and dashboard are now live with data for 12 long term conditions across the 4 UK regions (England, Scotland, Wales and Northern Ireland).
