Primary Care Networks (PCNs) were established by NHS England to support delivering care around small groups of GP Practices in local areas.
The dashboard can be used by primary care colleagues to ascertain prevalence rates for individual PCN Practices (as well as PCN groupings of practices) and compare these with other Practices and PCNs in their NHS (or ICB) areas. Data is available for a range of 20+ long term conditions and diseases including diabetes, COPD , asthma, dementia, hypertension, stroke, epilepsy, chronic kidney disease, depression, obesity and many more.
The dashboard can support PCNs in their efforts to deliver population based care initiatives and support initiatives to reduce health inequalities in primary care.
